The Endocrinology of Ovulation & The Menstrual Cycle
Ovulation is the biological release of a mature oocyte (egg) from an ovarian follicle into the Fallopian tube, where it remains viable for fertilization for approximately 12 to 24 hours. The menstrual cycle comprises three distinct biological phases:
- Follicular Phase: Begins on Day 1 of menses. Pituitary follicle-stimulating hormone (FSH) stimulates follicle maturation while rising estrogen thickens the endometrial uterine lining.
- Ovulatory Surge: High estrogen triggers a rapid surge of luteinizing hormone (LH), prompting the dominant follicle to rupture approximately 24 to 36 hours later.
- Luteal Phase: Following ovulation, the ruptured follicle transforms into the corpus luteum, secreting progesterone to prepare the endometrium for potential embryo implantation. Unlike the variable follicular phase, the luteal phase remains biologically constant in most healthy women at 14 days (±1 day).
Why the Fertile Window Lasts 6 Days
Although an ovulated unfertilized egg survives for at most 24 hours, healthy human spermatozoa can survive in the alkaline cervical mucus of the female reproductive tract for up to 5 full days (120 hours).
Consequently, the physiological fertile window encompasses the 5 days preceding ovulation plus the day of ovulation itself. Clinical studies published in the New England Journal of Medicine indicate that the highest probability of conception occurs during sexual intercourse 1 to 2 days prior to ovulation, ensuring viable sperm are already present in the Fallopian ampulla when the egg arrives.

- Cervical Fluid Changes: Fertile cervical fluid resembles raw, stretchy egg whites and can stretch several centimeters between fingertips without breaking.
